Handicapped Parking Permit Application

The City of Syracuse Department of Public Works issues handicapped parking permits (rear-view mirror hang-tags) to severely disabled city residents. A handicapped permit application must be filled out completely and signed by the applicant's physician. You may obtain this application on-line by clicking here or calling us at 448-CITY (2489). The application may be submitted to either: DPW, 1200 Canal Street Extension or at City Hall Commons, 201 E. Washington St., Room 102
